Sunday Editorial


The last few weeks have reared the ugliness and immaturity that exists in our country and in spine, this has become indicative by the vulgarity and sophomoric humor (to some) that has surfaced on TSB's blog. So let TSB share some wisdom, the definition of freedom means exemption from "control, interference, or regulation." If you notice TSB put those words in quotations. The time has come to implement regulation. Contrary to what some think, this is not your blog, neither is this a forum to spew vulgarities and non-sense..  You don't control the blog, neither do you have a "right" to spew the non-sense that comes out of some of your foul mouths. The purpose of this blog has been to provide everyone with a forum that no other platform has or ever will provide for the industry. You don't see it on OrthoWorld, Orthopedics This Week, or Orthospine News.  If the intent of the few is to destroy the blog or drive readers away, the necessary measures will be taken to incarcerating your thoughts or opinions. Censorship is a beautiful thing.  If you want to spew your non-sense develop your own blog, or better yet log on to Medical Holocaust, but then again no one follows or reads that second rate blog because no one likes crazy people. Yes that's right, CRAZY PEOPLE. Your rights and freedoms are nothing more than a fleeting moment.  So in closing, like the Times, the Wall Street Journal, The New Republic, or any other publication, it you want censorship remember what TSB says, "So I say to you, ask and it will be given to you; seek and you will find, knock too hard and the door will be slammed in your face."  TSB will not tolerate the disgusting and vile non-sense that the few of you write, neither do our readers welcome this aberration. Patients, Physicians, Public Officials, Attorneys and the General Public read this blog. If this vile and disgusting language does not cease TSB will block your IP address so you never have access again.  TSB wants to know what our readers think about these few individuals from North Carolina, Alabama and California that exemplify the unprofessional people that have infiltrated this industry.  This is your blog and TSB wants to know what you think,  It's time to let these people know what you think!

    1. Plus Blogspot.com only allows you to mark a comment as spam or delete the comment. The only thing you are able to see is the state/county from which your traffic comes from and not individual ip addresses of commentators.

  8. I apologize for making a few dirty comments although they were all retaliatory to suck it boy. Still doesn't make it right and I will abstain from sinking to that level again. I just can't stand it when people say things inaccurate or derogatory to others, it is the competitor in me and why I enjoy sales. I also enjoy the blog, the information provided and the healthy debate and would prefer not to have my IP address blocked. Although I would probably be more productive.

    1. Yes, but then you will limit the amount of information. Whether you like it or not Anonymous posting allows for facts to seep through. You have to sift through the BS but you are more likely to arrive at the truth if identities are protected.

    2. At first, I agreed with Evan. Then I realized that I would not be able to post anything on this blog because of my position. I have never said anything bad about anybody on this blog. I have complained about others saying gross, untrue, and harmful things anonymously that they would never say otherwise. Clearly removing the anonymous option would stifle comments, it would also raise the bar. TSB: I'm not sure how you can censor the blog given the about of time it would take on your end. But, it would be a great improvement if you eliminated the slander and profanity from the blog.

  14. Block Fat Bastard, EL and now Bally Baldez and anybody else who simply uses the anonymous postings to say things to people they are too coward to say in person. I also think anonymous posting should not be allowed, but rather by a user name. Maybe require people to become members and log in in order to post. Just do something, this has gotten ridiculous and nothing worthwhile about spine has been discussed in weeks.

  16. I agree with TSB, But you better do it quick before the spine industry in the whole is over. I can't stand the distributors or reps who think they earned all their business. It makes me laugh to hear how hard these indaviduals worked to get the business they have from their Father, Brother in-law, husband, 1st cousin, ect. I have been in this industry for a decade and it makes me sick or am I just a player hating what the game has turned into?

Why shouldn't we bitch, we use to sell on features and benefits, surgeons didn't have their hands out every time you went to see them, at least then you could see them. You have to laugh when a surgeon's practice demands that you pay to sell them something, how about Vaccaro's practice, they book a lunch and then ask for the distributors Amex Card, I'm sure that TJ would love to know that, but then again they may advocate that type of practice, hucksters, charlatans and then you wonder why people bitch. Could have happened with me sitting on the couch with my bong but I JUST GOT LUCKY after 100 hr weeks, travel to smelly cadaver labs, hundreds of rubber chicken dinners, hiring/mentoring/training reps, negotiating with the incompetent blobs in Mat Mgmt, fighting PODs. I'M SO LUCKY THANKS TO OBAMA BEING MY DEAR LEADER because I didn't build that, the DMV did.

    3. 90% of spine surgery patients would not classify themselves as victims. I track patient outcomes and I assure that a majority of patients gets their lives back after surgery. The surgeons that get the best results are far busier than the hacks, which shows patients do their homework.

  20. It is true that the nepotism has pretty much destroyed a solid business. It's not what you know, it's who you are married to, but whatever.... It is what it is... No one if forcing anyone to be in spine. Sure it's not what is was, and never will be again so deal with it. Maybe the government will have their say about PODS, but most likely they won't. Oh, and no one is going to jail, so you can stop wishing that on your enemies. I've been in this biz for 20 years. There are always crooks and 99% of them get away with it. It's just part of the sleazy side of med dev.

    So to get back to normal programming, does anyone know what's really going on at Synthes-DePuy? Are the legacy Synthes/DePuy reps getting the squeeze? Are reps jumping ship or thinking about it? Are people signing the non-competes?

    1. So far retention has been good out in the West...that is, after Armageddon going direct. Looking forward to new relationships to leverage with new products to sell. Those that stick are worthy and those that leave and whine didn't have the relationships or skills to make it. There will be a reduction in size, but it will be more of a Darwinian process than "deselection." Good look, but good riddance. Too many passive, "me too", farmer reps. This industry needs a paradigm shift and this acquisition will help.

    2. Those with skills and relationships should leave and work for themselves. It is much easier to sell with the 1000 pound J&J gorilla in your corner, but not nearly as lucrative. You are selling yourself short if you are such a great rep and still stay.

    I would block the fools that consistently post - we need a new topic, boring......if they would stop whining and use that effort and recommend a new topic we'd all stay attentive and informed.

  24. There are so many comments on this blog about "professionalism" yet the lack of professionalism shows with all the nasty, foul languaged, and truly immature comments. The spine industry has become more difficult every day and I don't propose that we all lock arms and sing Kumbaya, but a true exchange of information would be refreshing and professional. There are enough outside factors that we have to contend with every day, it would be great if we could use this blog for what I believe it was truly intended, an EXCHANGE of information not a place to launch personal attacks. Hmmmm, sounds like the image of our current presidential campaign.

    I'm just wondering if its really as bad as it seems for reps in different markets at the newly formed big boy. Or are they just blowing smoke?

  29. I'd be interested to learn from the posters and readers thoughts on how to covert just one surgeon customer from scratch. For those who have the business I've gathered:

    1. You either own and have owned it for many years. The relationships have been built on years of quality service, dependability and reliability...with corporate resources such as meetings, dinners, activities (things of the past).

    2. You've inherited the business. Perhaps the stable of surgeons utilize you because of the company you represent.

    3. You're related.

    What about building from nothing. New to a territory with little to no resources; can't even pull from out of pocket because it's against company policy to even buy drinks. Office staff don't know you and don't want to know you, not mention treat you like scum of the earth and OR staff work against you. Hospitals put you in the never ending hamster wheel of getting product approvals. All for a straight 8% on commodity products that everyone else has. What are the odds of success?? 1 year...maybe 2 a few surgeons will evaluate your product because they're pissed at the current rep for some reason or use you as a last resort.

    Clearly I'm not a rockstar rep yet and in a mid-sized city almost a year into this meatgrinder. Do I really have to align myself with a top five company or start pulling from saving to fund 'educational' activities? I'm all about whatever it takes at his point.

    It's no question to me why shady business transpires to get the business. Makes a person wonder what the hell is going on when the products you're representing are just as good and if given the chance would blow the service levels through the roof. How do you not feel desperate when you've got to provide for your family?

    If anyone responds, get out of the business, you're a douch. I'm just curious if any ethical hard working reps out there have had to start from scratch in the last 2 years and how they did it.

    1. Gotta tell ya bud, I would not want to enter the spine market at this stage as a newcomer. You have to ask yourself, why would a surgeon change products if he is facile with his current system and has a good relationship with his rep? To be successful in your position, you have to have a product that is unique or be with a company that is established and has a complete line. But even a rep with a top 2 company in my areas can't get a case because his company has never established a foothold around here and there are too many good, long term reps that control the business. If you can get into 2nd place with a few docs, then you might get some business when a product fails or a rep screws up or the company doesn't participate at the captivated prices, but you have a long row to hoe.

      The best best for a new rep, outside of having a truly unique product, is to get on board as an ASR for whoever has the market share in your area. Eventually, they will split the territory and you will be considered, but chances are that business was converted someone other than you and it will be vulnerable if the your relationship with the doc is not solid by then.

      All of this info is excluding the coin operated docs and the reps who feed them. For them, it doesn't matter what product you have. You can go that route, but know your business is always vulnerable to the next guy who is willing to feed more quarters into the machine.

    2. only way to do it is to sell a brand the doc recognizes but doesn't currently use. discount the crap out of it to make procurement give you a chance then slowly raise the price over time while you develop a relationship with the doc. kiss his ass, follow him to the bathroom with a roll of toilet paper if you have to. compliment his wife and shine his shoes for him when he changes out of his scrubs.

  30. Find niche products and work your way in that way. Commodity products will make it next to impossible. Be a resource to the physician and see what they are looking for, and not just what you are selling.

    1. 7:22 has the best advice. There are holes in everyone's armor. Buddy up with nurses and staff and work on finding out what areas a docs current company may not be doing a great job with. A lot of docs will bow to niches an keep 90% with one company. If you diversify yourself with 10% of multiple docs, you'll be doing OK, but more importantly you'll slowly earn their trust and cut further an further into the other reps share of cases. It's a long and grueling road, but that's how I did it. Good luck.

    Love "coin operated". Been in spine 20+ years and just got the shake down from a surgeon that expressed great interest in one of my products BUT it would cost me a 15% kick-back into his pocket to secure the business. He's a vending machine and apparently I'm supposed to be an ATM with a future cell mate. Looks like I picked the wrong week to quit drinking.

    2. I find it interesting to read the disclosures during some of the bigger meetings SRS, NASS, CSRS. I can see why some of these guys have deals, because they do bring a level of expertise above the norm. For instance, a guy like L.L. has a level of data and experience that benefits the entire community. While Joe Blow Surgeon down the road wants the same deal with much less experience and data collection.
      I have sold in a territory that 9:10 Surgeons have their hands out. In fact, have gone so far to say, "If my name is not associated with the product, I will not use it." Meaning, if he does not get paid to design, consult or research the product he won't use it.
      I do see a trend with the younger academic guys to buck the trend and stay above board with deals. I know of one guy that has turned down a ton of big dollar deals. We shall see how long that will last. I figure as soon as he takes a deal, I will know this industry is done for me.

  33. I appreciate this blog. Of course a few juvenile idiots lessen the mission, but remember you are in sales in an industry that rewards regardless of intelligence. Sales has never been selective. Medical device sales had it's grand run. If you weren't in on it, don't be a hater, it was timing. The industry is eerily similar to the mortgage industry. Both attract talent motivated by money. I bet many of you came from there. No fault in that, you are opportunistic. However, this ship has sailed too, unless you have flexibility to meet your customer's needs, which today is certainly not features and benefits of commodity products, you too soon will be wondering why you experienced a reduction in force.

  34. 10:36 - Thanks for asking a meaningful question and opening up constructive dialogue. I whole-heartedly agree with 7:22 and have made a 23 year career out of selling new niche products. It takes a while to get surgeons to change to a new mind-set so it is not as easy as a lot of commodity guys selling for the 800lb gorillas may think. You really have to change the way people think by helping them see that they are not accomplishing their objective with the commodity they have been using. The real challenge is finding the niche technolgy. It used to be a 6 year cycle where there would be something new and cooll. It would take 18 months to create a buzz then an 18-24 month solid run before others copied it or it got acquired, then the cycle would start over again. Now, the cycle is much longer than 6 years and there don't appear to be any new cool niche products surfacing on the market. TSB hit it on the head with the comment on plasma sprayed peek and titanium. Is this really a game-changer?

    What product(s) do you think is a solid niche technology? Does anyone see an unmet need that one could be developed for?

    1. We have to start thinking about positively impacting patient outcomes not re-invention of the wheel with certain widgets. I am sure the expandable cages have a small niche market, were as the cage helps facilitate a better outcome due to the design. However, that is more than likely a small segment of the actual usage that is being done.
      A "Game Changer" to me, is a product or procedure that significantly benefits the patient outcomes in a way that is clearly quantifiable. Think of USS staple system and the impact that made in the market and more importantly patient outcomes. I would appreciate hearing of any new procedures or products that accomplish patient outcome goals.

  38. Thank you to all who responded with words of encouragement and suggestions on surgeon conversions from scratch. It's professionals such as yourselves that inspire young blood! Interesting perspective on the slot machine analogy. Quite appropriate if I don't say so myself.

    Given the opportunity to promote niche hardware is closed at this point in time, the biologics arena may server as a more efficient strategy to get in the door and start working on a 2nd place position.

    By the way, how is it that unrestricted grants are not viewed as kick-backs in the industry?

    1. Research grants are kick-backs! These new companies think they are being clever, but when you boil it down..it's clearly a kick-back.

    2. @10:25 New Companies are not the biggest contributor to Research Grants. DePuy/Medtronic/Stryker are always in the top of the charts for grants. Fact is every company does it and in some cases it is very legitimate. An Unrestricted Research Grant given to a Academic Institution through the proper University channels is, in my opinion, legit. Unless you have the NIH giving you research or the University is making it rain with funds, the research does not get done.
      It is possible that you are thinking about those industry white paper "research" studies? I would agree that those are certainly in most cases kick backs. I see no reason to do a study on why company A's pedicle screw is better than company B's pedicle screw in pullout strength. Most of those studies are written by the companies research arm and "signed" off by the contributing Surgeon author. I feel sorry for any Sales Representative using White Papers to justify the superior function of their products.

    3. There are new company's poping up that pay surgeons to "evaluate" their products by providing reseach grants. They are paying for a surgeons business plain and simple. Those are the comapany's that are at the same slimey low level as POD's!

Were as the bigger companies stay mostly with high volume implanters or influencer. Reference the coin operated doc post. Those guys flip with the wind. Not worth the time or effort.

  40. I hate to be a D@#K, but I think this industry in a whole is in the can. I would love to expose distributors and reps that have customers that use there products because of their unethical relationship. This can be a family member(son, brother, sister, wife,cousin,brother in-law ect...) or company that gives cash payment. My question to TSB and his followers are. Who in your territory has a doctor using there products bc of this type of relationship? It would be nice to go back 10 years and sell based on benefits of product and how they would help the doctors and the patients. IF WE CAN EXPOSE THESE REPS/DOCS MAYBE WE CAN LEVEL THE PLAYING FIELD!!! (Or am I just bitching bc I don't have a family member that is an MD.)

      Carolina Spine and NeuroSurgery use Medronic products and after a little research I will post the personal information of an NC doctor who took nearly $200k in one year for speaking and travel while still doing surgery providing I have MSM's permission to do so.

      Based on 353 votes 72% of Americans would like to see the heads of the corporate medical industry tried for their crimes and shot for treason.

      The numbers are quite telling. 10% thinks that the FDA is doing a good job and that the medical industry is ethical. 10% is the same percentage that thinks the Republican congress is doing a good job. Obviously 10% of Americans are very stupid. 90% of Americans think that the medical industry sucks.

      Any industry that would put metal on metal joints in a human being is evil and that is why 72% of my readers would like to see those criminals executed.

      The US pays over 20% of it's GDP for health care. The countries with Government run health care pay 6% of their GDP for health care so your statements are completely fact free but then again you are a CON and information is like poison to you.

    10. You have no idea about European healthcare. France just stopped paying for ALL artificial discs, including your beloved discs, because of the cost. If a European wants to be seen and treated in a timely manner they buy secondary insurance and pay out of pocket.

      The U.S market is subsidizing the rest of the worlds healthcare. Because companies get good margins here they are able to sell cheaper in Europe.

      You are wrong about who pays taxes. 49% of Americans pay ZERO income tax. Everyone needs to pay something into the system which would make people more responsible and improve self esteem. The liberals want low income people relying on the government so they have guaranteed votes. That is the biggest CON in modern history. Most Americans would rather collect handouts than take the jobs the illegals are willing to take. If we take away well fare people will take the jobs , curbing illegal immigration, and at the same time provide for themselves and their families. The self worth of the U.S would rise significantly.

      You will see that I graduated from a prestigious university with high honors. I learned spelling in grammar school.
